Event Description

This presentation will explore the results of a comprehensive study that assessed the implementation of digital navigation programs across primary care clinics serving marginalized populations in California. Attendees will gain valuable insights into the successes and challenges of these programs, with actionable recommendations for improving digital health access. Digital navigators, clinicians, and health system leaders will benefit from learning how to better support patients in overcoming barriers to virtual care access.

Sarah Rahman, MD, MPH

Alameda Health System, ACMIO

Sarah Rahman is the Associate Chief Medical Informatics Officer at Alameda Health System, a safety-net provider dedicated to the health and well-being of Alameda County’s community. Since stepping into this role in January 2022, she has worked passionately to leverage technology in healthcare, with a focus on improving access and quality for underserved populations. With over a decade of experience as a physician informaticist, she supported Epic systems implementation and served as the Medical Director of Informatics and Virtual Medicine at her previous organization. Dr. Rahman is deeply committed to advancing digital health and promoting health care access, consistently advocating for the patient’s voice as new technologies, including artificial intelligence, continue to shape the future of healthcare

A graduate of Barnard College and Cornell University Medical College, Dr. Rahman completed her residency in Internal Medicine at UCSF. She is board certified in Internal Medicine and Clinical Informatics. In addition, she earned her MPH from UC Berkeley, with a concentration in Health Policy and Management, where her interests focused on digital literacy, telehealth, and AI in healthcare.
