Direct to Patient Workflow Considerations

1. Patient screening for appropriateness for telehealth

• Is staff screening the patients?
• Are patients self screening?

2. Will the patient be able to self schedule these appointments or do they need to speak with a staff member? How is the patient connecting with your office to request an appointment?

• Through telephone?
• Through a patient portal?
• Through online scheduling?

3. Will telehealth appointments be sprinkled in with your providers in person visits or scheduled in blocks of time?

• For example, will you block your provider’s schedule every day from 8-10am for telehealth?

4. Will you provide a pre-test call with the patient prior to the appointment? Who will conduct the pre-test call?

5. How will the patient connect to the telehealth appointment?

• Through a patient portal?
• Through an emailed link?

6. How will patient consent and any paperwork be collected prior to the telehealth appointment?

7. How will patients be reminded of their upcoming telehealth appointment?

8. Will staff be checking in with patients at the start of the visit or will the patient meet with the provider as soon as they log in?

Specialist to Clinic Workflow Considerations

1. Patient screening for appropriateness for telehealth

2. How will the patient or provider request a specialty consult?

• Does the patient need a referral? Or can the patient self schedule?
• Does the patient request through a patient portal?
• Does the provider submit a request to the specialist?

3. Whose office will handle scheduling the patient’s appointment once the request has been made?

4. Will telehealth appointments be sprinkled in with your providers in person visits or scheduled in blocks of time?

• For example, will you block your provider’s schedule every day from 8-10am for telehealth?

5. How will patient consent and any paperwork be collected/sent to the specialist prior to the telehealth appointment?

6. How will patients be reminded of their upcoming telehealth appointment?

• Is your office reminding patients or the specialists office?

7. Which staff will be rooming the patients at the start of the visit?

• Is a presenter required to be in the room during all or part of the specialty consult?
• If a presenter is not required, how will the patient or specialist bring a staff member back in to the room if needed?
