SUMMARY:NCTRC Webinar - Integrating Frontlines of Care and Specialty Services: Digital Health Tools and Autonomous AI As the Missing Link
DESCRIPTION:[vc_row][vc_column][vc_row_inner][vc_column_inner][vc_custom_heading text=”Event Description” font_container=”tag:h2|text_align:left|color:%2378a23f” use_theme_fonts=”yes” css=”.vc_custom_1692314796264{padding-top: 0px !important;padding-bottom: 0px !important;}”][vc_column_text css=””]This session explains how the strategic use of digital health tools and especially autonomous AI can streamline chronic disease management and improve outcomes. We focus on diabetes management and the autonomous diabetic eye exam as real world case studies at scale. As healthcare systems face increasing pressure to expand access and reduce costs\, digital modalities will no longer operate in silos but will converge into integrated care pathways. It allows frontlines of care such as primary care clinics (FQHCs) to focus on chronic disease management in its totality across all organ systems. Autonomous AI and other digital health decision support tools applied regularly to each of these patients\, ensures that those\, but only those\, patients that stand to benefit from specialty care for a complication\, will be identified\, scheduled and followed through via telehealth or in-person care . Attendees will gain insight into how these tools work in tandem to transform care delivery into a continuous process and shift the model from reactive treatment to proactive prevention.[/vc_column_text][/vc_column_inner][/vc_row_inner][vc_row_inner css=”.vc_custom_1692314355424{padding-top: 1em !important;}”][vc_column_inner width=”1/3″][/vc_column_inner][vc_column_inner width=”1/3″][vc_single_image image=”49714″ img_size=”full” alignment=”center” animation=”rda_fadeInRight”]REGISTER HERE[/vc_column_inner][vc_column_inner width=”1/3″][/vc_column_inner][/vc_row_inner][vc_custom_heading text=”Presenter:” font_container=”tag:h2|text_align:center|color:%2378a23f” use_theme_fonts=”yes” css=”.vc_custom_1692314247697{padding-top: 0px !important;padding-bottom: 0px !important;}”][vc_row_inner content_placement=”middle” css=”.vc_custom_1692310176984{padding-top: 1em !important;}”][vc_column_inner width=”1/3″][vc_single_image image=”63109″ img_size=”full” alignment=”center” style=”vc_box_border_circle_2″ border_color=”blue” css=””][/vc_column_inner][vc_column_inner width=”2/3″][vc_custom_heading text=”Michael D.
