We focus on diabetes management and the autonomous diabetic eye exam as real world case studies at scale. As healthcare systems face increasing pressure to expand access and reduce costs\, digital modalities will no longer operate in silos but will converge into integrated care pathways. It allows frontlines of care such as primary care clinics (FQHCs) to focus on chronic disease management in its totality across all organ systems. Autonomous AI and other digital health decision support tools applied regularly to each of these patients\, ensures that those\, but only those\, patients that stand to benefit from specialty care for a complication\, will be identified\, scheduled and followed through via telehealth or in-person care . Watzke\, MD Professor in Retina Research\, Professor of Ophthalmology and Visual Sciences\, Professor of Electrical and Computer Engineering (ECE)\, Professor of Biomedical Engineering (BME) Sciences at the University of Iowa\, with a joint appointment in the College of Engineering\, a fellowship-trained retina specialist\, computer scientist\, and entrepreneur. He is an IEEE Fellow\, and an ARVO Gold Fellow.  In 1989-1990\, he did a postdoc at the RIKEN neural networks research lab in Japan. As an expert in machine learning and image analysis\, Dr. Abramoff was one of the original developers of a widely-used open-source image analysis app\, ImageJ. His research has been continuously funded since 2004 by National Eye Institute\, the Veterans Administration\, the Beckman Foundation and other federal\, state and philanthropic funding agencies in the U.S. and Europe. \nDr. Abramoff is the Founder and Executive Chairman of Digital Diagnostics\, the Autonomous AI diagnostics company that was the first in any field of medicine to get FDA clearance for an autonomous AI. As the author of over 300 peer-reviewed publications in this field\, he has been cited over 35\,000 times\, and is the inventor on 17 issued patents and many patent applications. Dr. Abramoff has mentored dozens of engineering graduate students\, ophthalmology residents\, and retina fellows. DESCRIPTION:CTRC has helped hundreds of health centers to successfully implement and grow their telehealth programs. The CTRC Implementation Roadmap curriculum was developed by staff with over 40 years of combined knowledge and experience in telehealth\, in collaboration with a diverse community of health care pioneers and innovators who have shared their insight with us over the years. Join us in this upcoming virtual workshop series! \nAssess-Define-Establish-Implement-Improve \nThis first session will focus on the assessment portion of the Telehealth Implementation Roadmap. A needs assessment helps an organization identify key unmet needs and devise effective strategies and approaches to meet them. The CTRC Telehealth Needs Analysis provides a clear understanding of the nature and scope of unmet needs as well as a sound foundation for planning. The initial clinical analysis helps to clarify objectives and shared expectations\, improve coordination of services and resources\, and provide a supporting structure for your program evaluation. \nThe areas explored during this workshop session will include: \n\nSpecialty service needs\nCurrent payer mix\nTechnical infrastructure\, including:\n\nBroadband\nEquipment\n\n\nStaffing resources and existing telehealth experience\nClinical\, technical\, and administrative leadership buy-in\n\nAttendees will be required to complete and share the results of their self-assessment with CTRC to register and participate in the following three sessions.
